چکیده
We have presented a case of asymptomatic internal carotid artery coiling resulting in devastating ipsilateral ischemic stroke that was treated by intracranial mechanical thrombectomy. Morphologic changes the arteries been reported to be associated with aging, female gender, and standard cardiovascular risk factors. Dolichoarteriopathies (DICAs) are not uncommon, presence tortuosity, kinking, or coiling, particular, can cerebral ischemia. an 88-year-old woman our institution for acute right-sided stroke. The patient given intravenous systemic thrombolysis without effect. In accordance institutional guidelines, she subsequently referred Angiography showed significant left occlusion middle (Fig 1). Multiple old emboli were removed from after several aspirations full recanalization M1 segment 2). However, because other comorbidities, patient’s status improved only slightly sequelae disabilities. Subsequently, complex examination did find any source embolization except coiling. present has illustrated untreated DICA is potentially life-threatening finding deleterious outcomes. appropriate treatment DICAs result good outcomes low morbidity mortality rates. despite success surgical reconstruction, therapy remained subject numerous debates owing lack multicenter, randomized, prospective studies.Fig 2Selective angiography showing (blue arrow).View Large Image Figure ViewerDownload Hi-res image Download (PPT)

Mechanical Thrombectomy for Acute Ischemic Stroke in a Patient Treated with Dabigatran
Newer anticoagulants such as dabigatran have been widely used for the prevention of ischemic stroke in patients with nonvalvular atrial fibrillation (AF). A 64-year-old man treated with dabigatran 110 mg twice daily for chronic AF presented with total aphasia, right homonymous hemianopsia, and right hemiparesis at 90 minutes after the last intake. His prothrombin time-international normalized r...
متن کاملAn ischemic stroke patient with free floating thrombus in carotid artery, successfully treated by open carotid thrombectomy: a first case report in Nepal.
Free floating thrombus in the carotid artery is a well-known phenomenon, though relatively rare. We present a case in which we performed open surgery and achieved successful retrieval of the thrombus. A 40 year-old male patient presented with ischemic stroke and mild left hemiparesis. Computerized tomography and magnetic resonance imaging showed infarction in the right parieto-occipital area. C...
متن کاملRheolytic thrombectomy of the occluded internal carotid artery in the setting of acute ischemic stroke.
BACKGROUND AND PURPOSE Acute thromboembolic stroke complicated by ipsilateral carotid occlusion may present both mechanical and inflow-related barriers to effective intracranial thrombolysis. We sought to review our experience with a novel method of mechanical thrombectomy, in such cases, using the Possis AngioJet system, a rheolytic thrombectomy device. METHODS A review of our interventional...
متن کاملAngioplasty and stenting for mechanical thrombectomy in acute ischemic stroke.
A large number of patients presenting with acute ischemic stroke have large artery intracranial occlusions, and timely recanalization of these occlusions often leads to improved neurologic outcome. Starting with the widespread use of IV tissue plasminogen activator, a wide variety of pharmacologic and mechanical methods have been introduced to improve vessel recanalization and clinical outcome ...
متن کاملMechanical thrombectomy in acute ischemic stroke: catch device.
BACKGROUND AND PURPOSE Mechanical thrombectomy presents today a promising alternative to traditional stroke therapies. Our aim with this study was to evaluate the safety and efficacy of the Catch mechanical thrombectomy device in the treatment of acute stroke and report the angiographic results and clinical outcomes. MATERIALS AND METHODS We performed an analysis of 40 consecutive patients wi...
